Anesthetized with Iso/Os via mask.  Maintained at 4%.  Plucked
feathers surrounding the L carpal wound. Prepped with betadine.
 Removed 3 feathers that had grown under the wound edges.  Debrided
the skin edges with tenotomy scissors and trimmed back the exuberant
granulation tissue that was present.  Flushed the site with sterile
LRS.  Closed the skin with 4-0 PDS SI.  Placed bilateral carpal
bumpers.

2025-106 was brought inside for wound care and laser therapy. The right carpus showed promising improvement, with skin coverage and minimal scabbing. The sutures were removed from the right. The left carpus has a moderately sized scab and was freshly bleeding (likely from being caught). The sutures were still intact. This area was cleaned with dilute betadine, then received laser therapy. To prevent maggots, it was elected to put on a carpal bumper. Mupirocin ointment was applied to the area, then a small tegaderm + telfa was applied. Two strips of foam tape were applied. Finally, a large tegaderm with no telfa was applied over top in attempt to keep the area dry. Afterwards, he was brought back outside.

Carpal wounds look quite good. The L carpus has a thin scab present, but appears to be granulating in well from underneath, sutures c/d/I. The R carpus is nearly healed, sutures c/d/i. As we are concerned about stress of continued housing indoors (decreased appetite, weight loss), we elected to move p to outdoor aviary. Put thin layer of SSD on carpal wounds, did not place any bandages/bumpers. Moved to FA1. Plan to recheck on 8/1.

Wound Care:

The carpal bandages were removed. The left carpus appears to be healing well and has healthy granulation tissue present. The right carpus has some edema and serous discharge. The sutures are intact. Laser therapy was performed (see below). Both areas were flushed with dilute betadine and had a layer of SSD cream applied. The left was covered with duoderm, but the right did not have duoderm applied to better allow it to drain. Both carpi then had a tegaderm+telfa, foam tape, and another tegaderm+telfa applied.

Next bandage change in 2 days.

Surgery time - 60 minutes

Wound debridement of right carpal wound. Wound site swabbed for
lab culture since this site has been surgically treated before
and resulted in infection. Local anesthesia of the wound and
a small surrounding area achieved via 0.4 mL 2% lidocaine diluted
with 0.1 mL of 8.4% sodium bicarbonate. Flushed wound with LRS
and cleaned with betadine. Sterile scissors used to freshen the
apposing sides of the wound to ensure promote healing. 4-0 PDS
placed in a horizontal mattress pattern to close the wound and
reduce tension on the wound. Mupirocin ointment applied topically
with a PTA. Wound bandaged with tegaderm+ pad and secured in
place with padded tape and finally a suresite bandage. Almost
identical but slightly more superficial injury/wound found on
the left carpal when bumper bandage was removed. Opted to surgically
debride as well. We followed the same protocol on the left carpus
as the right except for several changes. We did not culture the
second wound as this was the first time surgical treatment was
implemented at this site. Further, during closure, 5-0 PDS was
used in a horizontal mattress pattern but due to high tension
the wound was left open slightly which we plan to close tomorrow.
This wound was bandaged in the same manner except for the first
layer we included a small piece of Duoderm to help provide scaffolding
for healing since the wound was left open partially.

Dx: Trauma vs cataract reabsorption vs chronic inflammation. It seems there is some chronicity, lens is not visible but it seems the anterior capsule is in place with pigment stuck to it.

Electrophoresis Comments
The alpha 2 and beta globulins are increased and reflective of acute inflammation. Moderate to
marked increases in these fractions can be observed with nephritis, hepatitis, dermatitis, and
mycotic infection. Increases may also be observed with sarcocystosis and chronic chlamydiosis as
well as mycobacteriosis. Specific beta globulin proteins which often manifest as a monoclonal
appearing peak in this fraction are also increased in egg laying birds. Followup EPH may have
prognostic value in this case.
